Hao Te Rangatahi

We provide primary care for rangatahi aged between 10 to 24 years old.

We also offer navigation and referral pathways to other services with the appropriate skills and resources within our community and beyond while walking them through their journey to a healthier, happier life.

Services we provide

The individual short and long term support services include the following:

  • Ongoing support for rangatahi and whānau

  • Health education and promotion to rangatahi and whānau

  • Identify clients and whānau at risk of illness or injury

  • Rangatahi-led - delivered wherever possible

  • Ensure rangatahi have access to healthcare that is youth friendly, comfortable, safe, high quality, evidence-based, and trusted.

  • Navigate rangatahi and whānau to other services they require.

Information and Education

Rangatahi and whānau are supported and enabled to manage their own health and health decision making through:

  • a greater understanding of health management

  • access to health services

  • reduce hospital admissions by providing effective primary care
