
About this Role
Kahungunu Executive, Rongomaiwahine Iwi Trust and University of Auckland are working in partnership to create a community-based aged care model for older and whānau in Wairoa and Māhia, delivered by local kaiārahi.
The purpose of the Kaiarahi role we will be involved in research to undertake a 'door-knocking’ approach to invite people in the community 55 years and older to participate. Through locally led engagement, kaiārahi from Kahungunu Executive in Wairoa and surrounding districts will:
Kōrero with older adults about their health, wellbeing, and social connections and complete questionnaires
Explore their goals, priorities, and aspirations for ageing well
Provide information about local health and social service providers
Support connections to health and social service provider
Key Responsibilities
Service Delivery
Door to door visits within a assigned areas.
Build rapport with residents
Complete consents and assessments (InterRAI, Hua Oranga and Huakina-specific questions)
Identify individual/whanau needs or risk and action goal setting and care planning
Collaboration and networking with services/agencies to increase individual/whanau access to other health and social services
Maintain privacy and confidentially of individual/whanau information.
Attend meetings when required
Staff Induction and Professional Development
Complete the KE induction programme.
Undertake training and development
Prepare for regular coaching with Team Coordinator.
Health and Safety
To be aware of and abide by the Health and Safety policies and procedures of Kahungunu Executive.
Reporting
To collect assessment feedback and data from clients/whanau about the service
What We're Looking For
Essential:
A relevant qualification/experience in health or health promotion.
Knowledge and experience in working with Whānau, Marae, Hapū and Iwi development.
Excellent interpersonal skills, with an ability to liaise confidently and professionally with a wide range of people and cultures
Excellent organisational skills, with an ability to cope effectively with changing priorities and environment
Good time management skills
Comfortable using electronic devices such as computers and tablets
A commitment to the Treaty of Waitangi and Tīkanga Māori
Sound written and verbal communication skills
Current clean full drivers licence
Police vetting check with no history that would require exemption
Experience in the use of all Microsoft suite applications
A willingness to undertake professional development/training relevant to the position.
Desirable
Proficient in Te Reo me ona Tīkanga
Knowledge of Health and Social Service providers
Be conversant with the Statutory requirements of the Privacy Act 2020 and the Health and Disability Service Standards
